This book summarizes and analyzes the possible distribution mechanism and pathophysiological mechanism of cerebellar subtonsillar hernia complicated with hydrocephalus by summarizing the clinical studies in domestic and foreign literatures. Although there is no exact theoretical basis for the specific pathogenesis at present, it has played a certain guiding role in the treatment of such diseases. Cerebellar subtonsillar hernia complicated with hydrocephalus is not uncommon in clinic, but the symptoms of hydrosyringomyelia or spinal compression are rare, and most of them are symptoms of a single type of disease, either with hydrocephalus symptoms or with cerebellar subtonsillar hernia symptoms. Clinical reports also mostly deal with one disease and rarely deal with two diseases simultaneously. Fortunately, the results show that treating only one pathological change can improve the results of two pathological changes at the same time, which reflects the importance of which disease to treat first, and the right choice gets twice the result with half the effort. Based on the analysis of domestic and foreign literature reports, combined with clinical cases of our center, the author believes that priority treatment of diseases with obvious clinical symptoms can obtain a better clinical prognosis. Whether it is cerebellar subtonsillar hernia or hydrocephalus, there is no clinical symptoms can choose conservative observation. In the case of unclear who is the cause and who is the effect, the first choice to improve the clinical symptoms of patients is the best choice for patients. Subtonsillar hernia after posterior fossa decompression is prone to secondary hydrocephalus, especially in the case of fluid accumulation under the flap. The specific pathogenesis is still unclear, but it may be related to arachnoid leakage and the formation of a one-way valve. Through the analysis of clinical literature, it is concluded that the treatment of hydrocephalus is still mainly shunt. Secondary decompression surgery or opening and enlarging the arachnoid does not always achieve the expected results. I hope this book can provide effective guidance for neurosurgeons.
